تحلیلی بر هزینه اعمال جراحی الکتیو لغو شده در بیمارستان شفا شهر کرمان بر اساس روش تحلیل هزینه در سال ۱۳۹۱ |

مقدمه: یکی از مهم ترین معیارهای تخصیص منابع بیمارستانی در نظام بهداشت و درمان، تجزیه و تحلیل هزینه های بیمارستانی است. بهره وری اعمال جراحی نقش به سزایی در تعیین هزینه های بیمارستانی دارد. هدف از این مطالعه محاسبه هزینه تمام شده اعمال جراحی الکتیو لغو شده بر اساس روش تحلیل هزینه در بیمارستان شفا شهر کرمان در سال 1391 می باشد. مواد و روش ها: این مطالعه توصیفی-تحلیلی بر روی 240 پرونده در یک دوره یک ساله 1391 به روش سرشماری جمع آوری گردید و با کمک چک لیستی محقق ساخته که روایی آن توسط صاحب نظران حوزه اقتصاد سلامت تایید شده بود، بررسی گردید. هزینه ها بر اساس 3 فاکتور پرداخت حقوق پرسنل درمانی، هزینه های منابع انرژی، هزینه های تاخیر در بازپرداخت شرکت های بیمه گر محاسبه و مورد تحلیل قرار گرفت. داده ها با استفاده از نرم افزار SPSS vol.18 و آزمون های آمار توصیفی و آزمون های استنباطی(تست دقیق فیشر) در سطح معنی داری P< 0.05 تحلیل گردید. یافته های پژوهش: از 240 مورد عمل جراحی لغو شده بیشترین دلیل لغو مربوط به وضعیت بالینی بیمار در بخش جراحی چشم(18 درصد) بود و مجموع کل هزینه های تحمیل شده 320 میلیون ریال بود که بیشترین هزینه مربوط به دستمزد پرسنل و پزشکان با مبلغ 69084800 ریال بود. بحث و نتیجه گیری: نتایج حاکی از آن است که دلیل عمده لغو اعمال جراحی در این بیمارستان به دلیل وضعیت بالینی بیماران بوده است. که شرایط مناسب برای عمل را نداشته اند و مدت زمان بستری آنان باعث تحمیل هزینه بالایی بر بیمارستان شده است. بنا بر این ارزیابی دقیق بیماران توسط پزشک مشاور و معالج می تواند از لغو اعمال جراحی و در نتیجه هدر رفت منابع مالی جلوگیری کند. |

Introduction: One of the most important criteria for allocation of hospital sources in health care system is to analyzing the hospital expense. Efficiency of surgical operations has an important role in determining of hospital expense. This study aimed to finished expense calculation of cancelled elective surgical operations based on expense analysis method in Kerman Shafa Hospital in 2012. Materials & methods: This a descriptive - analytical study was performed by consensus method on 240 files during a period of one year in 2012 and it was investigated by using a self-designed checklist that its validity was confirmed by the health economics experts. The expense were calculated and analyzed based on three factors including payment of medical personnel, expenses of energy resources and the expense of delay in repayment of insurance companies Data were analyzed by SPSS18 and descriptive and inferential statistics (Fisher's exact test) were used at the significant level P≤0.05. Findings: Of 240 cancelled surgical operations, the highest rate of cancellation was related to clinical status of the patient (%18) in the Eye department and the total imposed expenses were 320 million IRR which The highest expense was related to the personnel's' and physicians' payments with. 69084800 IRR. Discussion & Conclusion: The findings indicated that the main cause of cancellation of surgical operations in this hospital was clinical status of the patients who do not have the appropriate condition of surgery and the time period of their hospitalization cause high expenditure for the hospital. Thus, exact evaluation of the patients through consultant and the attending physician can avoid the cancellation of surgical operation and consequently waste of financial sources. |
