عنوان فارسی ارتباط مصرف منابع غذایی فروکتوز با شاخص های تن سنجی در بیماری کبد چرب غیرالکلی: مطالعه مورد-شاهدی
چکیده فارسی مقاله مقدمه: با توجه به افزایش روز افزون ابتلا به بیماری کبد­چرب غیر­الکلی(NAFLD) و به سبب توجه ویژه­ ای که در سال های اخیر به قند فروکتوز به عنوان یکی از عوامل خطر ابتلا به این بیماری و چاقی شده و نیز به دلیل کمبود مقالاتی که به دریافت این قند از منابع طبیعی آن-نه شیرین کننده­ های مصنوعی- پرداخته اند، مطالعه حاضر با هدف ارزیابی ارتباط مصرف منابع غذایی فروکتوز با عوامل تن سنجی در NAFLD طراحی گردید. مواد و روش ها: مطالعه مورد-شاهدی حاضر بر روی 57 بیمار مبتلا به NAFLD (تایید شده با اولتراسونوگرافی و سطح آنزیم­ های کبدی) و 57 فرد سالم همسان شده از لحاظ مخدوشگرهای احتمالی چون سن، جنس و نمایه توده بدن(BMI) در کلینیک شیخ الرئیس تبریز صورت گرفت. شاخص های تن سنجی به روش استاندارد اندازه گیری و BMI، دورکمر به دور باسن(WHR) و دور کمر به قد ایستاده(WHtR) محاسبه گردید. با استفاده از پرسش نامه بسامد­ غذایی، بار مصرف هفتگی میوه­ ها و سبزی ها و با استفاده از پرسش نامه ثبت غذایی 3 روزه مقدار فروکتوز دریافتی و سهم آن از انرژی و درشت مغذی ها به دست آمد. یافته های پژوهش: میانه بسامد هفتگی مصرف میوه ها و سهم فروکتوز از انرژی در کل بیماران و نیز میانه بسامد هفتگی مصرف میوه ها در زنان با چاقی شکمی به طور معنی داری بیشتر از افراد سالم بود. گرچه فروکتوز کل دریافتی با BMI در افراد سالم همبستگی مثبت معنی داری را نشان داد(r=0.273, P=0.040) ولی تفاوت معنی داری از لحاظ میانه بسامد هفتگی مصرف میوه ها و سبزی ها به عنوان منابع طبیعی فروکتوز در سطوح مختلف BMI مشاهده نشد. پس از کنترل عوامل مخدوشگر تنها عامل پیشگوئی کننده این بیماری WHtR یافت شد و سایر عوامل نقشی در بروز NAFLD نداشتند. بحث و نتیجه گیری: یافته ها حاکی از عدم وجود ارتباط منابع غذایی فروکتوز با عوامل تن سنجی در بیماران NAFLD می باشد.

عنوان انگلیسی Association between ÷Dietary Sources of Fructose and Anthropometric Indices in Non-Alcoholic Fatty Liver Disease: A Case-Control Study
چکیده انگلیسی مقاله Introduction: Regarding the increased risk of Non-Alcoholic Fatty Liver Disease (NAFLD) and due to the special attention to fructose as a risk factor for developing NAFLD and obesity in recent years and also due to the lack of papers that have worked on fructose from its natural sources -no artificial sweeteners-, this study was designed to determine the association between dietary sources of fructose and anthropometric indices in NAFLD. Materials & methods: This case-control study was carried out on 57 patients with NAFLD (confirmed by ultrasonography and elevated liver enzyme levels) and 57 healthy subjects matched for age, sex and body mass index in Sheykh-ol-rais clinic in Tabriz. Anthropometric indices were measured by standard method and BMI, Waist to hip ratio (WHR) and waist to height ratio (WHtR) were calculated. Using a food frequency questionnaire, weekly consumption of fruit and vegetables were estimated and using 3-day food records the amount of fructose intake and its proportion of energy and macronutrients was estimated. Findings: The median frequency of weekly fruit consumption and total fructose from energy intake in all participants and the median frequency of weekly fruit consumption in women with abdominal obesity were significantly higher than healthy ones. Although the total fructose intake and BMI in healthy subjects showed a significant positive correlation (p=0.040, r=0.273), but there was no significant difference between weekly median frequency of fruits and vegetables consumption as natural sources of fructose and different BMI levels. After adjusting for confounders, only WHtR was a predictor and other factors have no role in the pathogenesis of NAFLD. Discussion & Conclusions: The findings suggest no association between dietary sources of fructose and anthropometric indices in patients with NAFLD.
